Please be aware that Dr Pravin Jain and Dr Anke Gisbertz will be leaving the Surgery on 13th November 2019. All patients registered with them will be allocated another GP after this date. In view of this, our List will close from 1st August 2019 to New Patients and we will be unable to accept any new registrations for 6 months following 1st August 2019.

Appointments are now available - ask our Reception team for more details. Please note however that for evening and weekend appointments, you may be seen at a different surgery and not by your own named GP, but the Doctor will have full access to your Medical Record.

HSCIC Data Extraction – Fit Note Data
All Practices have received a communication from HSCIC about the extraction and publication of fit note
data. The extraction is intended to allow the DWP to collect data on fit note usage in order to inform
policy development and evaluate the Fit for Work service, and the data will be fully anonymised.
This information will be published by this Practice on our noticeboards and on our Website.
or on their noticeboards providing links for patients to obtain further detail. Data will not be extracted for
those patients for whom a Type 1 objection has been recorded.
Data will be collected in February (to include fit notes issued from December 2014) to be published in
spring 2016. Aggregated data will be published on the HSCIC website at CCG level and above.

Monday to Friday between 8am and 8.30 and between 5.30pm and 6.30pm there is a new out of hours number to dial - 0845 145 0121. After 6.30pm and at weekends you can dial 111 for assistance .

- NHS 111 directs patients to the service that is best able to meet your needs taking into account your location, the time of day you are calling and the capacity of local services.
When should I use 111?
- When its not a life threatening situation and, therefore, is less urgent than a 999 call
- When you cannot see your own GP, for instance when it is closed or your are away from home
- When you feel you cannot wait or are simply unsure of which service you require
- When you require reassurance about what to do next
