The prescription system via this website has been discontinued, please contact the Surgery via email to firstname.lastname@example.org and you will be issued with a username and password for future use of our Online Services"
Please note we need to send your Prescription to a nominated Pharmacy as we will be transferring to sending all Prescriptions electronically to a nominated Pharmacy. Therefore please include a Pharmacy of your choice - we will stop holding prescriptions at the Surgery for collection very soon. All prescriptions will be sent to the Pharmacy electronically as we become paperlight.
Prescriptions for medications ta ken regularly can be obtained without seeing the doctor, these are called repeat medications. Requests must be in writing, i.e. completion of the repeat slip on the back of the prescription, a letter to the doctor, or completion of a form at reception.
We do not take prescription requests over the telephone.
Please note if there is an alteration on your repeat slip, or you are requesting additional medication not listed on the repeat slip, your prescription will take longer to process.
Patients on repeat medication will be asked to see a doctor, nurse practitioner or practice nurse at least once a year to review these regular medications and notification should appear on your repeat slip. Please ensure that you book an appropriate appointment to avoid unnecessary delays to further prescriptions.
Please allow three full working days for prescriptions to be processed and remember to take weekends and bank holidays into account, also if there is a query or an alteration to you medication your prescription will longer to process.
