Nurses offer appointments here at the Surgery to discuss travel health risks and offer basic travel advice.  However, we would like to advise patients that they may be asked to attend a Travel Clinic if more specialist advice is required. The Practice Nurses have limited training in Travel Health; therefore it may be necessary to direct patients to obtain further information.

Please complete the form below in full, hand it in to the Receptionist at the surgery for the Nurse to view it, and then call after 5 working days to speak to a Receptionist who will then make you an appointment with the Nurse. This will either be a 10,20 or 30 minute appointment depending on your needs and the destinations you will be travelling to.
