New Patient Registration
Unfortunately due to 2 of our GP partners leaving Bolton Road Surgery, we are closed to new registrations. We will however register family members of existing patients who live within our boundary, also new born babies and adopted children.
The CCG are aware of this and we would like to apologise for any inconvenience this causes. However with only 2 GP partners working, we have to maintain a safe working environment.
If you wish to register, and we have agreed to take you on to our list, you will need to complete a new patient registration form which asks for photo and address i.d and details of your past medical history. You will also be asked to attend for a new patient check with the nurse to which you should bring a sample of urine for testing.
Each of the doctors at Bolton House have personal patient lists. You will only see another doctor if your own doctor is on leave. We will allocate you to one of the doctors within the practice.
Please note patients are not entitled to use the car park attached to the surgery when attending for appointments, this car park is for the use of Staff only.
Medical treatment is available from the date of registration. Please contact reception for further information.
